Been taking a combination of OxyContin 40mg 2x daily new formulation for three years along with 30mg IR Oxycodone for break thru pain 2-3x daily. Out of completely nowhere I go in and my dr discontinued the 30mg IR OXYCODONE all together and I was in bad shape he switched me to the same quantity OXYMORPHONE 10mg IR and I get no pain relief so I went in again and said it did not work so he said he will put me on 15 mg IR morphine sulfate 3x daily for BREAKTHRU and since I have to wait for that to be approved thru insurance he put me on Dilaudid 2ng three times a day and it's not working at all should I stay on Dilaudid and ask to raise the mg or should I just try this morphine IR AND POSSIBLY BE MISERABLE since I suffer from chronic pain do to a spinal thoracic lumbar fusion and a second surgery that was failed and has me in ten times more pain than I've ever been in. Idk what to do. Any advice would be appreciated thanks

So, apologies for an entire paragraph qualifying what I'm trying to say to a post more than 1 and 1/2 yrs ago!!!!!. Chucky, plain and simple, my PCP is a Dr who doesn't think his patients should have to suffer pain. His' motto' is 'Not On My Watch!'...and no I'm not giving out his name or location. 4+ yrs ago (?..just not sure on exact timeframes? ) he decided to add a REFORMULATED narcotic - Opana.- to my pain mgmt plan (folks don't seem to remember or just don't' know that Opana was killing people right and left in the early to mid-90's so it was yanked off the market for a good 15 yrs maybe --? and I could be ALL wrong about the dates of all of this however the story is true!!.)
So, my Dr added Opana (now at 40 mg ER, one every 12 hrs) and reduce Oxy's aka Roxy's from six, 30mg IR pills every 24hrs to three, which is still pretty hefty but is working beautifully for me. I walk, I talk, I play, I work and feel great in general. But this isn't suppose to be about me. It is suppose to be about drug comparison. My PCP told me that the equivalency between a 20 (not 40) mg ER Opana, twice daily and the oxy's/roxy's was 180mg. 20mg of ER Opana equates to 180 mg of IR Oxy. Opana at 20 mg ER, 1 dose, = 180 mg of IR Oxycodone. (I just want to make sure I'm clear on that)
This info is/ was only as good as the powers that be thought at that time. Why is it that no-one will step out on that limb in today's world and just give a simple comparison? Is it because it's really not that simple? What IS the answer? I would really like to know. Many thanks for the time and thought you and anyone else may put into any response!

I suffer from VERY SEVERE chronic pain. [I was hit by a semi truck, while driving on the highway, and I'm perm disabled.] I take Opana ER 30 mg twice daily & can Tage up to 6- 10/325 NORCOS for break through pain. I don't get any narcotic side effects, which is fine with me! All I want is to not be in pain & this is the ONLY combination that works for me. I hope this helps!

After being on different meds for 8 yrs or so, I feel like the oxycodone helps. I take oxycontin 80 mg. These are the only ones that seem to help but not much helps anymore, so I am going for the surgery.

I was put on Opana 20 extended release; 1 pill every 12 hrs.. I was already taking up to 6, 30 mg a day for Oxy-30 instant relief. My Dr at the time told me the 20 Opana was easily equal to 3, Oxy 30 if there was a way to get past the formulations serious tamper proofing (as I understand it now, its easy to cut them in half but the reformulation still works as a show release.) The Opana instant release (to take the place of the oxys) is harder to get and I don't have any info on that except I have heard people say the ultimate goal if you are not one of those who is really hunting for the euphoric high and you seriously just want to stop hurting all the live long day, have your doc give you the 20 XR Opana with 1 pill every 12 hrs with a 5mg IR Opana up to as many times a day as s/he will. Or a different IR like 10mg Oxy's (??) not sure if those are oxys. They are small round and pink....for break through.
Summary: I started with 2, 5mg/500 vicodin up to 4 times a day then 7.5 / 325 2 a die up to 4 times daily. Then the pinkies (10mg-IR) pinkies' up to 6 times a day then the 30mg oxys (up to 6 times aday...stone and they actually started to amplify the pain!!; then the 20mh XR Opana with several daily 30mg oxys. Now I'm on a balance that is working best for me. 40 mg XR Opana every 12 hrs plus 45 oxys per month to use as needed, 30-mg oxys as needed.
Believe it or not i often times do not need the oxys at all for days on end so as far as I'm concerned the next step will be reduction of some sort until I am down to 20-mg EX Opana and maybe 3 pinkies (really sorry....just don't remember the name!!). No clue if this was helpful or just another diatribe. I'm a proponent of feeling as good as I can with a clear mind. Hope what I've written has been some help; its mostly a hodgepodge of possible combinations....and you just want equivalency. Good luck!

It sounds as if I try the insuffilating method, I may get better pain control. Correct? Or, at least, a break from the agony.

Bryan, You talked about the Fenanyl patch lasting 72 hours. Then you wrote ( I think it's 6 hrs or less). What did mean by that last remark. I use Fenanyl and I love it. Doc your last remark refer to time each dosage in released? I was told each dosage is released every hour.

I am currently on Oxycodone 15mg - 4x daily. I have had several surgeries and just started treatment in a pain clinic. This medicine is not completely helping my pain. My dr. told me that she would give me some long term meds this next visit if the current medicine didn't help. Oxycodone works well for me but not at the current mg. My question is this, in the State of Tennessee, can I get long term meds ( Opana) if my dr. ups my Oxycodone to 30mgs - 4 times daily? Just trying to find the right mix for pain treatment. Also, where do I find the "scale" used to help with my question? Thanks
