Insurance Prior Authorization And Morphine Equivalent
UpdatedIt's memorial weekend and my dr. sent my rx to the pharmacy but insurance is asking for prior authorization. I have been taking morphine sulfate er 100mg for 5 years twice a day. I have 30 mg immediate release. What would be the equivalent to take?
