I have my appointment with pain management tomorrow. I am on fentanyl patch 25 every 48 hours and norco 10/325 four times a day it isn't working and my quality of life stinks any suggestions what I could be switched to. Since it has to be done when I see him.

Do the fentanyl patches make you feel funny like the oral narcotics do sometimes.. Sort of euphoric. I kinda creeps me out. I do better on the oral meds or liquid oral. Anyone else??
This is true the insurance refuses to pay for the 48 hours so o have to pay for the 3rd box at $125 per box. And it is true the longer and higher dose you go to stop 100% of the pain your brain will cause the pain to come back and you up the strength to get back to 100% and in the end you will max out at 100mcg every 48 hours and still be in pain. Better to suffer 20% pain 90% of the time and hit it harder at bed time so you can sleep. I am now suffering at 50mcg and using ice and heat during the day and then taking the 1 to 2 Norco at bed time. And 1 to 2 to help me get up. Then from 10 am till 9 pm I am at 20% to 30% pain level.
Glad he adjusted it for u...I haven't heard of a Crack down type thing..but things r alot harder always when it comes to us trying to just do things right..sometimes they follow thru sometimes not i guess..he is doing u a solid so to say by giving it to u over 2 days instead of 3 I kno some that won't don't that..but I've heard that type of thing said before and things can go on same as is for a long time or it can change..idk how it will go but for now I hope it stays this way for u as long as possible..just keep fingers crossed n glad he is working with u he seems to get it..where if it did change it wouldn't be his call so much as someone elses...time will tell..but good deal for now bud..
Hey bud (PainSux), thanks for all the advice. I just saw my doc today. He went ahead and adjusted my 100 mcg patches for every 48 hours - gave me a new script for 3 boxes. But, he stated that the gov't/insurance companies are starting to refuse those scripts. Since the patches are not "officially" designed for 48 hours, they are starting to enforce 72 hours. Even though there's proof that these patches wear off after 48 hours for some people, they are still being pricks about it. He said it's all about the $$. The pharmaceutical manufacturer's couldn't get the script approved by the FDA unless they could guarantee the patches were good for 72 hours, so the fudged on the facts. Even though it's common knowledge they wear off sooner, the gov't is now about to enforce to 72 hours. That is so wrong! So, I will see how much longer my insurance provider will cover it for me. Have you heard about this yet? Or is my doc giving me a line of bull?

Hmmm...yes I do see what u r saying...n telling him that u get less needs to be point made first...then I would ask if since u do ok for the first 2 days but not on the 3rd if he could possibly do the same thing as b4 a do every 2 days..doesn't hurt to ask...just tell him the truth n that it is a less amount anyways overall...n if it was a 2 day thing would be fine and that for you that 3rd day REALLY doesn't work good and I is a miserable 3rd day everytime...not just once or twice but all the time...n would it be OK in HIS opinion cause that 3rd day I horrible for you everytime...n see what he says...let him kno that it's fine n then just stops working. And if he could manage that for u? Atleast see I guess is my best opinion to give to u bud? Thanks, and very good points! Yes, I can see where the doc needs to (feel) like they are the ones in control. If I were to tell him my adjustments then that would be like disobedience to him. Better to let HIM make the adjustments. I have appt with him tomorrow morning...but not to increase to 100mg, because he already did that last week. But...he increased me from 75 to 100s, but he decided to make me keep the patch on for the full 3 days instead of changing after 2 days. I didn't realize it until recently that, what he did was actually prescribe me LESS medication overall! 3 boxes of 75 mcg is 1,125 mcg. 2 boxes of 100 mcg is 1,000 mcg. He said he would increase the dose, but didn't want to make it for only two days because that was too big a jump all at once. NOW WHAT? Since last week I have NOT been able to keep the patch on more than 48-50 hours. The pain and severe discomfort creeps in and it's sheer agony. So NOW I have to go see him tomorrow to explain that, even though he prescribed the patches for 72 hours, I had to change them every 48 instead. (like I WAS doing with the 75s). I will need one more box of 100 mcg to get through the month, but not sure how he is going to deal with THIS now. lol! What a nightmare!!

If I was u I wouldn't go in and say I'm cutting them up and this n that some may see that as trying to take control when the docs need to feel like they r in control n they r but I would say if I was u the truth...that u rnt getting enough relief and hurt alot still it's helps some but not enough to feel good enough..n ask for the 100 to get that little bump that u seem to need becuase u still hurt...just my thoughts but I think either way u day it the main thing is what ur on isn't working and the idea is to get it to where it is...n I hope he/she sees that...good luck
Why did you need to stop your medication? Did the source of your pain go away, or was it cured? I'm only asking because I have DJD condition and am currently prescribed Fentanyl 75. However, I'm finding that is not enough for my pain, and I am cutting one up into quarters to add to my pain relief. I regularly use one full patch, plus one quarter now. But, now even THAT is not enough for my pain. I think it would be easier if I could just get my doc to prescribe the 100 patch, but I haven't asked him yet. I'm afraid he will be angry that I have had to resort to cutting partials for my pain thus far. But, it's NOT to feel high, it is legitimately for some serious PAIN.
One more thing. Do not try to eliminate ALL the pain...your senses will be too dulled if you do, and the requirement will only increase. Learn to tolerate SOME pain.
I'm on Fentanyl 50 every 3 days. Norco-10 6X/day became ineffective after 4+ years. It's nicer to only have to change a patch every 3 days than to have to pop a pill every 4 hours. My suggestion is to get off the pills completely. Sleep becomes a bigger and bigger issue...or should I say/write lack of sleep as you begin to awaken in pain in 2 hours or less, in need of another pill.
Try to increase to Fentanyl 50 MCG/Hr. and drop the pills.

I speak from years of experience and I think you are on the right path. Run don't walk away from a higher (and higher) dosage of Fentanyl. I too started with a 25 and went up to the 100. I am having a devil of a time this hour trying to rid my body of that stuff. It is very, very hard to get off Fentanyl once you are physically dependent on it. Good luck.
@Ishmael I feel that is a very good point you explained very well. You re not implying, accusing any1 of anything. Evryone who takes opoids becomed dependent ,but at 1 time or another I know I stop and put myself in check and with my breakthrough meds for example see how many I can just NOT take and at times when I go to take 1, I remind myself to either stretch, bathe use ice or heat for my back. Really in the end i'll only be hurting MYSELF...very good question to ask one's self. sounds as if you knew some1 who turned to china herion? (my bad not your words but) seems as if you are either speaking from1 who went through or saw some1 go throughthis!! (my opinion) Best of wishes to you
Fentanyl is really the strongest thing available, but there is a stronger dose available at 50mcgs and the patches can be changed every 48 hours, rather than every 72.

I agree with you about the diff. btw pain relief and a buzz. I have been on my current dosage for 5 years and consider that to be good. The injections are very good also, no buzz, so you are right. I was assuming that the op was not just seeking a higher dosage. I wonder when people want off the patches for oxy's as they are easier to abuse and sell. I am glad you got yourself cleaned up, way to go.
Is it the fact that your pain is still bothering you, or is it that you aren't getting a buzz from the dose?
Don't take offense, it's just a question - this is how you decide if you're dependent on pain medication or if you are an addict. If your dose is too low to take away the pain, you're dependent. If you're doing it for a buzz, you're an addict.
That is pretty much the standard, across the board definition. And you, and only you, are the one the can answer that question truthfully. If you are looking for a buzz, you're going to get higher and higher doses and sadly, my friend (and I know from personal experience) that you'll keep increasing the dose - up to $400/day of China White heroin dosed with fentanyl - 50 bags from Philly, and - at the end, even that stopped working.
Not trying to be the moral police, not trying to talk you out of it - just trying to tell you if you are looking for higher doses, because you need relief but you need a buzz, then be careful - it will ultimately end badly, and while you tell everyone you 'lost' everything you ever had; but the truth is, you will have given it all away.
That is, if you survive it and get to a poiunt where you're ready to own the truth and share that story.
I would be surprised if your pain doc. didn't put you a higher patch. The 25mg is just a starting dosage to see how you tolerate the drug. I was on a 25. every 72 hrs. plus narco's as needed, then upped 50mg every 72hrs. Now am on 75mg. every 72 hrs. plus narco's as needed and get good pain relief from that plus twice yearly steroid injections into my spinal cord. There is not much stronger then the fentynal, they are stronger then oxy's and if you don't get relief there is not much else out there, but you are on a low dosage. Good luck.
