No, this does contain 37.5mgs of controlled release Paroxetine. The tablet should be round, orange and have writing in black that has an M over a P5 on one side. Your pharmacy has not made an error.
What led you to believe that this wasn't the correct medication?
Paroxetine is an SSRI antidepressant, the active ingredient in Paxil and it may cause side effects, such as nausea, dizziness, weight changes and headache.

It was causing side effects that matched Prednisone to the tee and my depression was getting worse. I think Mylan has screwed up. I need to get these pills analyzed. I got my doctor to call in the name brand and I am feeling much better now!!
Why would they even change the generic when it was working just fine the way it was? There is something wrong with that new generic, I'm telling you NOW!!
I called the manufacturer and reported the adverse side effects and they said they would make a "Adverse Side Effect Report" for themselves and pass it on to the FDA. I wonder if they really did so!!

These ones aren't actually new to the market and haven't been changed. It is, however, normal for your pharmacy to switch which generic it carries, depending on whatever company was offering the cheapest price, when they had to restock their supply.
As to why it might have affected you differently, that could have been due to the allowable differences in generic drugs. To get them to the market faster and to keep prices as low as possible, they are allowed to differ in the amount of the active ingredient by as much as plus or minus 20% of the amount in the name brand product.
The means that you may not have been getting as much of the medication as you were, when you were taking the medication from a different manufacturer. That would explain many of the symptoms you were suffering, including the worsening depression.

As to an incident report and complaint, you can call the FDA and make one yourself by calling their MedWatch number at 800-332-1088.
There is, however, no way to have them analyzed, without your paying for it.
