I have had pcod since 2009. I am always having irregular periods also got hypothyroidism. Since the last 3 months I am having little bleeding daily. I consulted with one gynecologist. She had given primolut 10 mg for 5 days, 2 times daily. Nothing happened until the 3rd day. From the 3rd day I started menstruation, with very painful cramps. I could not get up from the pain. At last I took panadol (paracetamol) for 2 days. The pain and cramping is now ok. But the bleeding is increasing with heavy clotted blood (like the size of a lemon). I am really got tired. Need to change my pad every 1-2 hrs. Today is my 4th day. Still I am having heavy bleeding. What I should do? Should I consult my doctor or wait for a week? Thanks.

Hi l need ur help l hv used implant fr one year bt eversince it was inserted in my body l hv never seen my periods what can l do to get them
HI, Maya! I'm very sorry about the issues that you've been experiencing.
The Primolut N contains a progestin Norethisterone, which is a synthetic form of progesterone. In many cases of irregular bleeding, such as the daily breakthrough bleeding that you were getting, your body isn't making enough progesterone on its own, this is very common with PCOD.
Estrogen is what causes the increase in the endometrial lining each month and it is the increase of progesterone in our bodies that stops it from breaking down, until the time that no pregnancy is detected, after that the Progesterone level drops off, it stops inhibiting the MMPs, they work to break down the endometrial lining and your period occurs.
Thus, if you're short in progesterone, there is nothing to keep that lining intact and inhibit the MMPs, so the lining can just keep continuously breaking down and that's what results in such irregular bleeding.
By giving you these tablets, your doctor was trying to supplement yours back to oppose the estrogen, so they don't just keep continually building the lining and to stop those MMPs from breaking it down all the time.
And what happens when the progesterone is added back like that is basically a uterine clean out, your body is now working hard to get rid of the excess lining that had built up, plus the blood and tissue that had not already been expelled, so that is a good thing.
However, if it continues for more than a week, or if it starts to get extremely heavy, then yes you should contact your doctor as more medical intervention may be necessary…..and of course if you're concerned, feel free to contact them immediately, it's your life and health that's of concern here, so you should always do what's right for you, regardless of what anyone online tells you.
In many cases, PCOD is found to have been caused by oxidative stress, meaning your body is lacking antioxidants and other vital nutrients to function properly. Adding them back, alongside a healthy diet and supplementing your progesterone levels can actually bring it under control and greatly improve your quality of life.
One nutrient of great benefit for preventing breakthrough bleeding like this is N-Acetyl Cysteine, since it also inhibits those MMPs to prevent them breaking down the endometrial lining.
But before you make any changes, please consult your doctor and make sure that it is safe for you to do so.
Is there anything else I can help with?
