I know this post is over a year old, but I've been dealing with chronic, intractable pain since the early 2000's. I recently obtained a new PMDR back in August 2013 who practices in the same office as my surgical specialists (cervical spine & shoulder). It was a blessing to find her and I am grateful. One thing she DID give me that no other pain doc ever did was the Fentanyl patch. I had one PMDR for almost eight years before he lost his license, and another for a couple months from January-April 2013, who was too far away and I disliked very much because of the way I was treated. The patch is specifically (as is written on the box) "ONLY for pain requiring opioid medicine around-the-clock." I started out taking 25mcg, with instructions to take two initially if the one did not work well. I have an extremely high tolerance (considering my size) and am NOW taking 50mcg. transdermal patches, replacing every 48 hours, instead of the usual 72 as it initially was prescribed. During my last visit I told her the patch didn't seem to work for more than two days, and since she told me she had heard the same from other patients, adjusted mine to every 48 hours, which was a big difference. I'm also taking two 10mg Percocet every six hours for breakthrough. However my tolerance to this has gone up since I've been on this same regimen albeit it Oxycodone or Percocet, which is the same except it has acetaminophen in it. She is willing to switch me back to the regular oxycodone so will discuss with her during my next visit which is Monday the 18th on dosage. I have been through hell between PMDR's but am not going to elaborate except to say I was forced to go through withdrawal on a couple of occasions that made me so sick I wanted to kill myself. As long as I adhere to the contract I now have with my current PMDR, I don't see any problem going forward. I feel very blessed to have found her when I did. For ME... I can't speak to anyone else, the patch allows me to go round the clock with enough relief I don't have to get up in the middle of the night to take breakthrough meds.

Due to the way the human body works, the answer to your question is never, it isn't possible for them to create one that will do that.

Your body will eventually get used to any thing that you ingest regularly, from a medication to your morning coffee and the effects it had at the beginning will start to wear off. If you want the same effects back, you'll always have to adjust the amount you take in, switch to something else, or take a break from it for awhile.

Since it's not the medication that actually does it, but your body, there is no way to overcome it and change it.

Can I help with anything else?
