The closest match I'm finding to your description is that of a white oval pill with the number 4 on one side. The other side is scored into four quarters with 42 in the top left corner, 16 in the top right corner, nothing in the bottom left corner, and it looks like a square root sign in the bottom right.
This pill is identified as Methylprednisolone 4 mg; Manufactured by Vintage Pharmaceuticals Inc.
