Sometimes, taking a medication on an empty stomach can cause it to be more effective. In most cases, that doesn't mean to take it a long time before a meal, but just before you start to eat.
The FDA lists the typical side effects as possibly including nausea, dizziness, headache, increased urination, lowered heart rate, and hypotension.
For example, I take Labetalol for blood pressure and the directions for it state that it can be taken with, or without food, but it has a higher efficacy rate if taken on an empty stomach, so you should be consistent, that way your doctor can get accurate readings and adjust your dosage accordingly.
There are also cases where you may be instructed to take a medication with food, because taking it on an empty stomach may cause too much nausea, and stomach irritation.
