Dilemma With 2 Narcotic Scripts From 2 Doctors (Page 3)

Hi all. I have intractable pelvic RSD, peripheral neuropathy, lumbar facet spondylosis. I live in California and am about to have to change insurances at the end of this month. The new insurance has ridiculously high Rx prices. I go to a PM for all my pain meds, but my primary dr has in the past managed them for me. My question is if anyone knows if I get an Rx for the 60mg Oxycontin twice a day like usual from my PM and then my primary to write a script for 40mg 3 times a day and then I can go to 2 totally different pharmacies and fill one with my insurance and the other at another pharmacy and pay with cash, will that flag the DEA and cause my PM to get a notification of "Doctor shopping"? I am just trying to have enough meds to get me through an extra month until my husbands new job let's us get medical coverage?

I had to do this one other time a year ago when the same thing happened with me loosing my job because I could no longer work and I wasn't married yet to get my husbands insurance. The first 2 months I had no problem, but the third and LAST month I was needing to do it to get me enough meds to last me the 3 month waiting period, my husband forgot and filled both scripts under the same insurance but just at 2 different pharmacies instead of paying cash at the second one. My pain doctor found this out and thank God I was able to explain my way out of it and he didn't discharge me as a patient, but now in this situation again, I'm afraid to try it one last time til we are settled with yet another health insurance. I would really appreciate anyone's advise or experience they could share regarding my situation. I truly am not a doctor shopper, I am just afraid to run out of meds and be in massive pain and / or risk getting into trouble.

TAD You should be ok,they're both different meds and mgs,my PM Dr at first was giving me a script for both 40 and 20 mg oxycontin to take 3x a day until I got bumped up to 80's.You should be ok.good luck

the pharmacies and the gov't track the distribution of a rx not just what your insurance will pay. I have been in the same predicament as you. and I asked my pharmacist and that is what he told me. unfortunately, meds that are working for people like you and I seem to be a problem for the gov't. the people who misuse these meds are the people who are ruining the care that we receive. my doctor has just told me that "the office" decided to stop giving these meds to her patients any longer. so she is letting her office board of directors dictate her treatment care for me and many others I have spoken to in the office. I am, needless to say, looking for a different doctor.

I'm just curious I'm curious about whatever this "flagging" is all about?
If they flag you is it FOREVER & only in MY state or what? Only once I had been discharged & I told my PCP that I was seeing a PM dr. SINCE I had 2 previous back surgeries & went in for my routine follow up visit for other issues medical issues he treats & I knew he was refilling my usual meds & yes I told him I was in pain but never mentioned I wanted anything for it because my pm dr already was prescribing it for me. It was his mistake because I DID NOT know he had sent it over through fax with my other meds & of course I don't dr shop because I use the same pharmacy for the same Drs! Then when I went up there there was two separate scripts for hydrocodone one for like 30 & the other was more so no I don't want to pick them both up of course & surely they wouldn't let me. That upset me because I really didn't know because if I did I wouldn't have used the same dang pharmacy!!! Then like a month later I get a certified letter from him dismissing me from his office for violation of pain contract. I was far to upset by then because my PCP screwed me & it was innocent. I felt that he'd think I was like most everybody else. My question is that will he still have me in his system after 6 years??? There's another dr in his office. He moved again to another location. Please help. I've had over 25 different surgeries for different things & I'm needing some relief now.

The info about why the dr dismissed you is in your medical records. When you see a new dr for the first time they have you sign a release form for them to get your previous medical records. You may have a hard time finding a dr to rx the same med or anything strong. It just depends on the drs.

Put Pennsylvania state board of medicine in your search bar. Their website should tell you if he is still licensed.

U cant do it bc all prescriotions for narcotics r tracked as of this year. Theres a website that tells ur doctor exactly what scripts were written AND when. Im a nurse in NY and this is nationwide

Maybe I am missing something but if your going to pay cash at the second pharmacy then why not just wait the 28 days and do it the legal way? You just said you can't use your insurance twice cause clearly that will get you caught again. But you said you were doing it cause you were not going to have insurance while he changes jobs. So since your paying cash why would you have to do it now? If your doing it to feel high then just tell us. I know thats why I would be doing it. Also to answer your question it depends on the state you live in and if they use a monitoring system.

It doesn't matter if you're paying with cash or insurance. The pharmacist has to check the PMP in your state. If it is too early, they do not have to fill it.

Hi Guys,

Stumbled upon this thread and wanted to provide some insight. I am a Pain Management Physician and these kinds of threads scare me. The reason controlled drugs are enforced so highly, is because they have a very high abuse potential, on top of the fact they are strong medications that can alter many things, apart from mood, diet, hallucinations, etc.

Lots of pharmacies share electronic records. If you get multiple prescriptions for the same or similar drug and try to avoid the system, you will get flagged sooner than later. The computer systems with medical programs have been catching up and it is not so easy any more. It is surely not worth getting caught for something like this.

This is what you need to do. You need to see a physician that is correct for your needs. If you just had surgery, your surgeon will prescribe the correct pain medications. If you are in chronic pain, a pain management physician can assess your needs and provide you with the correct amount and dosage needed for the prescription amount. Do not try to get multiple Rx's from different physicians at this stage. You are risking the physicians license by ingesting more than the known quantity to him/her of that said drug, and you are also harming yourself by possibly taking a dosage larger than safe. Reconcile all your files with one prescribing physician.

You should only see a family physician for controlled substances, IF, you have been long term on the said controlled medication, AND, your family physician is comfortable with writing you the said controlled medication(they may not be and don't blame them). Controlled medications have high abuse potential as known. When consumers abuse them, you risk your life, our license and the integrity of the Rx system.

Best way, explain to a Pain physician what you need exactly, to a tee. Only go to a family physician if he/she is willing to write you that controlled prescription based on your forwarded records. Also plan frequent appointments to check your vitals and important criteria to make sure these drugs are not taking a toll on you. If you avoid these frequent check ups, you will not be aware, unless you are well read on all the side effects of the drugs, how low or high other hormones/neurotransmitters/vitals can go.

Please everyone, do yourself a favor, once again, visit one physician, get all your medications for that specific need from only that one physician.

Also I have a Question if anyone can help. I don't want this to be a problem or look bad on my record, I am a long time chronic pain patient and my PM doctor retired himself my medication was only last until 7/29(3days ago) i saw few new dr but not successful and only one wrote few 5mg Oxycodone! For emergency which i didnt get it filled yet(i was on oxycodone30mg x5daily & Opana40mg x3 daily. Ifound a new PM doctor and my app is in 2days now that I'm out is that ok if I get those 5mg oxy so I can make it to my new PM doctor or it will mess up my record for the new doctor? The way im thinking im not the new pm patient in the new place yet so its not like im having 2 pm dr at the time and im just getting the 5mg once to make it to my first app in new pm dr but I'm worry if still it affect on my record or look bad in front of the new dr ? Please let me know whats the right thing to do now. Thanks

Your old PM doctor should have set you up with a new PM doctor that would continue your current treatment plan. I've had this happen to me once before, but my retiring PM doctor set me up with a PM doctor that didn't try to reinvent the wheel... The bad thing with the PM industry is that there isn't any uniformity with how and what doctors prescribe their patients. I started PM in 1989 and now in 2014 it's still like the wild west.
In all those years I've been around alot of different people with different PM doctors and nothing is etched in stone.
All I can say is always be honest with your PM doctor about meds from all doctors and pharmacies used. Most make you sign an agreement to use one pharmacy, but things happen and you might have to use a different pharmacy. Always notify the PM doctor that you had to use a different pharmacy. Some PM doctors will fire you for taking any narcotic meds from any other doctor or even a dentist even though it has nothing to do with what their treating you for, even though you kept them informed. This type of PM doctor you want to stay away from. I actual know a guy who was in PM and broke his arm on a Friday evening. The emergency room doctor gave him 30 10mg Norco after they set his arm and put it in a cast, he told the PM doctor about it on Monday and the PM doctor fired him. Sounds ridiculous?, but it actually happened...
With your current situation all I can say is use your same pharmacy, make sure those 5mg Roxicodone aren't overlapping any other narcotic pain meds that have been prescribed to you. Inform the new PM doctor of everything pertaining to your situation. Be completely honest, but don't go overboard on the honesty. Going on and on can make you appear like your trying to convince them of something.
Seeing several different PM doctors may make some PM doctors think your doctor jumping to get drugs your after, because drug seekers do this. Unfortunately this day and time alot of PM doctors are paranoid about drug seekers. Seeing a new PM doctor can be like starting from scratch, they don't know you and they don't know if they can trust you with schedule II opiates. Be patient, don't insist on certain drugs first time around, be open to trying what the new PM doctor wants to do and understand your developing a new relationship with a stranger who doesn't know you at all nor you them. Eventually you will get what you need to have some quality of life(hopefully), but it's more than likely not going to happen up front.

Thank you Mark for the good information, it was supposed to be like what you said! he retired himself gave the last month supply and referred me to new dr but when I went there he chose to 100% stop all the pain medication i had suddenly after 9 years and said i didn't promise to your pm dr to give you any pm medication !!! Not even lower it in few month thats why i had no choice but to try another dr and another but as you know its not about your injuries nor your records once you go to a new PM dr its like starting from scratch or even worst ! Im sorry for your friend and I had the same experience 2 years ago i end up in ER and they gave me some pain killer for the small surgery I had and what I did tried to be honest with my PM dr and told them the story and even that i have the left over available for them to trash it if they chose to but my honesty made me get discharge from them ! Maybe i could just not mention it and that would not get me fired from the pm dr i was going but its all that you don't expect & not whats logical its what they say even on weekends that you have no way to call your PM dr to ask them before your normal treatment of whatever case happened to you! So I learned to be honest but not too honest once I find one good PM dr i would only use them and only one pharmacy, honestly its a war for us who really need Painkillers to have a normal life i was lucky ( i was almost about to give up) to find this new PM dr and I could actually discuss everything on the phone before my app and I almost know what is she going to write for me instead of just go there and leave empty handed! Btw the stupid dr(AKHTAR PURVEZ) from hope clinic that left me with absolutely no medication and expecting me to stop in 24 hours!!! Got in trouble for doing that ! I found out he was promising to patients for a good treatment just to collect their money and send us home with nothing he was doing that to many others so they reported him to The board and he's no longer in the office to ruins peoples life. Anyway good luck and I hope you dont have to deal with the stress of not being able to get your medication and I hope My app goes well too! It's actually tomorrow morning and I hope things goes as i was talking on the phone with her! Thanks all and wish you all a life with no pain.

I think as long as you explain everything to your general practitioner, you should be fine. Tell him you were trying to be honest and it hurt more than helped. After all, you did have two molars extracted. That certainly warrants extra pain med. Best of luck to you and let us know how it goes.

I was seeing a pm dr many years ago. He was no longer accepting my insurance so I needed another dr who would continue to write the same scripts. My pm dr gave me a letter stating his office no longer accepts ...... insurance. He listed the meds n dosages he had me on n in short, wrote I was a good patient n followed his orders without any issues.blah blah blah...basically a letter of recommendation as a pm patient. He also gave me a 30 day supply of meds to get me thru while I searched for a new dr. As far as filling 2 scripts at tbe same time with different payment forms...ur going to screw urself in the longterm. There is a new national database that scans ur name n info...the DEA has been cracking down on narcotic pain meds. Hense the reason alot of pharmacies are turning away pain meds and choosing to not fill a narcotic drug regardless of how long uve used their pharmacy. Is it worth chancing your long term pain management plan? Especially if u fill ur rx at Walgreens or CVS. ...theyve been hit hard by the DEA n now theyre walking on pins n needles...

Thank you all for all the great information, in short you don't know how mess is out there until you try to change your PM doctor, i had my first visit with this new PM dr that already accepted to treat me and it went well i guess The small amount of Roxycodons(5mg) didn't affect on anything and the new PM did cut some if my meds but since she changed one of my meds( Roxycodone30mg to Opana 10mg IR) I'm actually happier with the result she was very understanding over all my problems & how to write the names of meds ( if Oxymorphone works better some pharmacy wont give you the right kind if your script say OpanaER, it has to say Oxymorphone or you'll end up with the new kind of Opana that is worst than a tylenol !!!

Also my old PM dr did set up a new pm dr abd few other names incase but when I actually went there at the time I supposed to the new dr had a changed heart and sent me ( including many other patients) home empty handed ! Yea changed his words and gave me nothing but gabapentin !!! And told me i'll be fine with my pain and stopping 120mg Opana and 150mgRoxycodone wont make me sick !!! He was an animal (Dr Akhtar Purvez) someone that everyone should stay away from
Anyway when he changed his promises I felt lost again and started looking around on my own to find a PM dr and it is not easy these days but after few wasteful visits I finally found one that accept to treat me good but with a little smaller amount of what I was getting and today I had my first visit and so far I'm happy with the result.
