I just had cataract surgery and am taking nevanac. I understand you have a rebate program. As I paid a great deal for this drug may I please have a rebate form. I live at 4460 Grafton Av N,Oakdale MN 55128.

The manufacturer has an assistance program for Nevanac, for patients that don't have prescription coverage.
