Building Up Tolerance

BEEN ON 30 MG OXYCODONE 4 A DAY N 15 MG OXYCODONE 3 DAY FOR BREAKTHRU.ALL R IM RELEASE.SHOULD I CHANGE THE 30 MG TO 60 OF MORPHINE ER.SINCE THE EXTENDED RELEASE WILL LAST LONGER N KEEP THE 15 FOR BREAKTHRU?

Swapping the Oxycodone for Morphine sounds like a good approach if you need to try something different for tolerance reasons. But with regards to potency, Oxycodone is said to be approx. 1.5x more potent than Morphine on a milligram to milligram basis. So I can't say for certain if 60mgs of Morphine ER will be quite enough to replace Oxycodone 30mgs. It's probably worth trying though since you'd have the 15mg Oxycodone for backup relief.

Is your doctor pretty flexible with you wanting to change things up?

First of all, 30 and 15mg Oxycodone tablets are not immediate release, these dosages are only available in the regular release formulation, which is a generic for Roxicodone, or in the extended released under the brand name Oxycontin. Thus, it seems you've been taking the regular release tablets.

That said, have you tried Oxycontin, which is the extended release?

As to the Morphine, if you did go with that, you'd actually need to increase the dose to be equivalent to the Oxycodone, since it's actually one and half times stronger than Morphine, so for example each 10mg dose of Oxycodone would require a 15mg dose of Morphine to be analgesically equivalent.
