I'm taking Dilaudid 4mg four times a day, and was prescribed Nucynta 50mg twice a day for breakthrough pain since Darvon was taken off the market. I'm not getting any relief from the Nucynta. Could you recommend anything else for the breakthrough pain that isn't an NSAID, which I cannot take due to liver disease? I sure do miss Darvon.

I also have Hep C so I got on the patch which is better for the liver, I would try to get on Fentanyl which works excellent and it is safe, and use you dillauded for breakthrough. If you are not willing to change whatever you do don't get on Oxycodon. They have such bad withdrawals and raise your tolerence really fast. Ask for tramadol or IR morphine.
I was put on Nucynta 50 mg last month by my pain management doctor. At first I felt absolutely no relief whatsoever either, but my doctor suggested I try taking 100 mg at a time to see if it made it any more effective. To my surprise it made quite a difference. I am now prescribed 100 mg of the ER(extended release) Nucynta and am having some positive results in treating my chronic pain for the first time in a while. I would ask your doctor what he thinks about this as an option. I hope you find something to manage your pain soon, nobody should have to live in pain every day if it can be avoided!
Do u find that the Nucynta makes your lower back and legs ache ?
There are quite a few options on the market to treat pain that do not contain NSAIDs.
Have you tried Tramadol? It is also a centrally acting analgesic, like the Nucynta, and is also non-narcotic, it is available by itself, without any NSAIDS:

There are also many other narcotics on the market, without NSAIDs that could be used to treat breakthrough pain, such as an instant release Oxycodone or Morphine:
