B12 injectible is 5 years past exp date. Is it still effective? Can it cause illness?

Yes, liquid formulations can actually go bad, once they are expired and it could make you sick to try to use them. It would be best to see your doctor for a new supply, if needed.

The FDA lists its typical side effects as possibly including nausea and administration site irritation.
