The identification that Mushuollie posted is correct. This tablet is manufactured by Teva Pharmaceuticals. It is used to treat depression and some severe cases of insomnia.

The FDA lists the typical side effects as possibly including nausea, dizziness, headache, dry mouth, mood changes, and weight changes.
