This is generic hydromorphone, (I'm about 99.9% sure), 4mg. The brand name is Dilaudid. It is a very very strong narcotic medication used to relieve severe to very severe pain. It can be very addictive and cause drowsiness, nausea and/or vomiting, decrease in appetite, sweating, blurry vision.
