If you've moved there, that is your state now, so the fact that you lived somewhere else prior to this shouldn't matter.

I moved to WA several years ago and don't tell people I'm a resident of my old state and my doctors sure Dont consider me to be from out of state.

You need to list Mississippi as your current address, leave your old state out of it and go on from there.

The fact that you're paying cash also shouldn't matter, as long as you can pay the bill.

Have you been having places refuse to see you?
